Gallstones have traditionally been considered a problem of middle-aged adults, but there has been a sudden surge in cases due to broader changes in lifestyle taking place around. From skipping breakfast to bingeing on fried and unhealthy food at midnight, young adults are unknowingly setting the stage for gallbladder issues. Identifying the underlying causes of gallstones and seeking timely medical help can significantly minimise unnecessary pain and discomfort.

Why Are Gallstones Targeting Young Adults?

According to Dr Nikhil Yadav, HOD, Minimal Access and Robotic Surgery, Manipal Hospital Dwarka, New Delhi, “Gallstones are hard, pebble-like formations of digestive fluid that can develop when bile hardens in the gallbladder. They can be asymptomatic and just sit there, but sudden, sharp pain, located in the centre/upper right abdomen, is a common symptom of gallstones.” They have always been common in adults, but the increasing prevalence in young adults can be due to the following reasons:

  • Low-fibre diets: Increased intake of junk and processed foods, along with unhealthy habits like frequently ordering in, has created a diet that is rich in fats but low in digestive fibre. This retards the gallbladder activities and stimulates the gallbladder stone formation.
  • Dehydration: Failure to consume sufficient amounts of water may make bile juice thickened and eventually lead to the promotion of the formation of crystals in the bladder. It is compounded in the hot weather of India.
  • Sedentary lifestyle: Office work, working remotely, and spending more time at the screen are some factors that create the situation of the absence of physical activity among young adults. This leads to inadequate flow of bile, and thus stones get more prone.
  • Hormonal factors: The prevalence of gallstones is also higher in women, even among the young adults. Hormone therapy and oral contraceptives put them under greater risk of being confronted with stones due to the influence the oestrogen has on the bile composition.
  • Metabolic disorders: Obesity, diabetes, PCOS, and insulin resistance have been on the rise worldwide and seen in those individuals as young as their 20s, closely related to the issues with gallstone development.

The rise in prevalence is also due to greater awareness and early detection. Ultrasound, which is the first choice for any investigation, has become more accessible. Many people now also opt for annual health checkups, where asymptomatic gallstones are often detected early.

How Are Gallstones Treated?

Gallstone treatment revolves around the size, number, and severity of the symptoms. In severe conditions where the pain levels are so high that they affect the quality of life, the physician may order a laparoscopic cholecystectomy surgery, as it is a very common procedure and it involves small cuts, minimal pain, and quick recovery. Dr Nikhil Yadav says, “A more recent and more enhanced procedure is robotic cholecystectomy, which is more precise, particularly when there are complex cases, and the same benefits apply in terms of speed of recovery and less scarring.”

In a case where the gallstone slips into the common bile duct, ERCP (endoscopic retrograde cholangiopancreatography) may be required to remove the stone. However, ERCP is not a standard treatment for gallstones inside the gallbladder; it is only used to remove stones in the ducts.

Ways To Prevent Gallstones

Gallstones may be silent and then all of a sudden very painful or nauseating, or they may even come up with issues such as infections or inflammations. The risk can be minimised with basic prevention measures, such as drinking a lot of water, having healthy nutritious diets, avoiding consuming a lot of oily foods, and doing physical exercises.
